Clinicopathological comparison of periapical cyst and periapical granuloma in a cohort of Tamil population

Aim: The aim of this study was to present and analyze detailed clinicopathological data of periapical cysts (PCs) and periapical granuloma (PG) in a cohort of 135 cases from the South Indian Population. Methodology: The present study included 135 cases of PC and PG out of 2696 biopsies submitted over 3 years. The clinicodemographic data which included age, gender, location, radiographic appearance, and treatment were collected along with the histopathological examination of the biopsied specimen. Data were entered in a Microsoft Excel spreadsheet, 2021, and analyzed using SPSS software ver. 26. Results: There were 71 cases of PG and 64 cases of PC. The mean age of occurrence in PG was slightly lower than cases in PC. Irrespective of the group, there was a clear male preponderance, and maxillary permanent central incisors were most commonly affected. However, no significant difference was noted. Radiographically, PC significantly showed more well-defined corticated radiolucent lesions compared to PG where most cases were ill-defined (69.01%). Histologically, all cases showed classic features for diagnosis with additional histological characteristics which may aid in diagnosis. Conclusion: PG was more common than PC. There was a predilection for the male gender in both lesions. The actual incidence of these lesions would be actually high, as some cases are lost to private practitioners, and not all the lesions are submitted for histopathological examination.

A study of macrodontia of the permanent maxillary central incisors among Delta State University students in Abraka, Nigeria

Macrodontia (or megadontia) is a rare dental anomaly characterized by excessive enlargement of all tooth structures, and in few cases may be associated with morphological anomalies. The goal of this study was to find out how common macrodontia of the maxillary central incisors was among Delta State University students in Abraka. The Research and Ethical Committee of the Department of Human Anatomy and Cell Biology, Delta State University, Abraka, gave approval for this study. A descriptive cross-sectional survey with a representative sample of 102 subjects was conducted (58 female and 44 male). The method used was a simple random sampling technique. The age and sex of each subject were obtained using a data form, and intra-oral measurements of the mesio-distal width of the maxillary right central incisors were taken. Results were arranged according to age and sex to ascertain the occurrence of sexual dimorphism; independent sample t test and chi-square test were used as inferential statistical tools. The prevalence of macrodontia was 35.3%; there was a significant sex difference, as p value was <0.05; there was no significant difference within the age groups, as p-value was > 0.05. The mean mesio-distal width was 8.54±0.25 mm in those who did not have macrodontia and 9.63±0.54 mm in those who did have macrodontia. Macrodontia of central incisors is dominant amid Delta State University learners in Abraka.

An in vitro evaluation of Icon resin infiltrant penetration into demineralized enamel lesions using an indirect staining technique with confocal laser scanning microscope analysis in dual fluorescence mode

Abstract Context: White spot lesion is the first clinical sign of a caries lesion and represents mineral loss from the enamel subsurface. Aim: The aim of this study was to evaluate the penetration depth (PD) of Icon resin infiltrant into artificially demineralized enamel lesions using confocal laser scanning microscope (CLSM) analysis in dual fluorescence mode. Settings and Designs: The design of the study was an in vitro study. Materials and Methods: 22 extracted human permanent maxillary central incisor teeth were collected, and enamel sections were obtained from the coronal middle third. All enamel specimens were exposed to demineralization and remineralization solutions for 14 days. On positive confirmation of enamel demineralization by scanning electron microscope analysis, 20 specimens were then subjected to Icon resin infiltration following manufacturer instructions. Specimens were processed with indirect staining technique using rhodamine B and sodium fluorescein dyes and examined under CLSM at ×10 magnification in dual fluorescence mode using ImageJ software to evaluate PD of resin infiltrant into demineralized enamel lesions. Statistical Analysis Used: Obtained data were analyzed using an independent t-test. P ≤0.05 was considered statistically significant. Results: The maximum depth of demineralized enamel lesion was 590 μm, and the mean depth was 290.78 ± 14.80 μm. The maximum depth of resin infiltrant penetration was 580 μm, and the mean depth was 279.08 ± 13.88 μm; P = 0.006. The percentage penetration of resin infiltrant was 95.99%. Conclusion: Icon resin infiltrant was highly effective in its depth of penetration into demineralized enamel lesions. The use of indirect staining and CLSM analysis in dual fluorescence mode is more reliable and accurate technique to evaluate the PD of resin infiltrant.

Dental Developmental Defects: A Pilot Study to Examine the Prevalence and Etiology in a Population of Children between 2 and 15 Years of Age.

Dental development defects (DDDs) are quantitative and/or qualitative alterations produced during odontogenesis that affect both primary and permanent dentition. The etiology remains unknown, being associated with prenatal, perinatal, and postnatal factors. The aims were to identify the possible etiological factors, as well as the prevalence of DDDs in the primary and permanent dentition in a pediatric population. Two hundred twenty-one children between 2 and 15 years of age, patients of the master's degree in Pediatric Dentistry of the Complutense University of Madrid, were reviewed. DDDs were observed in 60 children. Next, a cross-sectional, case-control study was carried out (60 children in the control group and 60 children in the case group). The parents or guardians completed a questionnaire aimed at identifying associated etiological factors. The prevalence of DDDs in patients attending our master's program in both dentitions was 27.15%. Otitis, tonsillitis, high fevers, and medication intake stood out as the most relevant postnatal factors among cases and controls. The permanent maxillary right permanent central incisor and the primary mandibular right second molar were the most affected; there were no differences in relation to gender. One out of three children who presented DDDs in the primary dentition also presented DDDs in the permanent dentition. Prenatal and postnatal etiological factors showed a significant relationship with DDD alterations, considered risk factors for DDDs in both dentitions.

Analyzing the Impact of Different Bonding Protocol Strategies to Improve the Fracture Resistance of Uncomplicated Crown Fractures on Tooth Fracture Reattachment in Permanent Anterior Teeth: An In-vitro Study.

Objective This study aims to analyze three different bonding protocol strategies in determining the fracture resistance on the reattachment of fragments in permanent anterior teeth. Methodology This study evaluated the ability of three bonding methods (Group A, total-etch technique; Group B, selective-etch technique; and Group C, self-etch technique) to enhance the fracture resistance of reattached tooth fragments. Sixty permanent maxillary central incisors were chosen, segmented at 3 mm from the incisal edge, and randomly distributed among the three groups. Tooth fragments were stored for 24 hours in GC Tooth Mousse (GC Corporation, Tokyo, Japan), and then reattachment was done using respective bonding techniques. Fracture resistance was gauged using a universal testing machine. Results The mean fracture resistance values were as follows: total-etch (419.5995 N), selective-etch (359.1448 N), and self-etch (192.0887 N). One-way analysis of variance (ANOVA) and post hoc Tukey tests revealed a statistically significant difference between the groups. It was inferred that the total-etch technique exhibited the highest fracture resistance, while the self-etch technique resulted in the lowest fracture resistance (P < 0.05). Conclusions The choice of bonding technique for reattaching tooth fragments should be made based on clinical context and patient needs. Total-etch provided the highest fracture resistance, but selective etch can be preferred for anterior teeth with lower occlusal loads to prevent sensitivity.The self-etch technique may not provide sufficient strength and should be used cautiously. More clinical studies are required to validate these findings and guide clinical decision-making in traumatic dental injury management.

Morphometric Comparison and Prognostic Analysis of Permanent Maxillary Central Incisors with Eruption Disturbances-A Cross-Sectional Study.

The aim of this study was to retrospectively compare the morphometrics of permanent maxillary central incisors with and without eruption disturbances, while simultaneously evaluating prognosis based on different factors. Seventy patients with unilateral permanent maxillary central incisor eruption disturbances were included. Within a group of 70 subjects, measurements were taken for both normally erupted central incisors and central incisors with eruption disturbances to determine the length of the roots and the volume of the teeth. Various factors, such as angulation of impaction, and vertical height of impaction, were assessed to investigate their correlation with surgical intervention. Both the root length and tooth volume were significantly smaller in the eruption disturbance incisors than in the normally erupted incisors (p ≤ 0.001). Moreover, there was a statistically significant increase in surgical intervention among cases with no clear physical barrier (primary retention) (p < 0.05) or when adjacent normally erupted central incisors exhibited more than 2/3 of root development (p < 0.05). The results of this study numerically demonstrated the delayed tooth development of the permanent maxillary central incisors with unilateral eruption disturbances compared to appropriately erupted incisors by measuring root length and tooth volume. The absence of obstacles and the degree of root development in adjacent erupted incisors might serve as factors for clinicians to determine the necessity and timing of surgical intervention.

Comparative evaluation of incidence of dentinal defects after root canal preparation using three different endodontic retreatment systems - An in vitro study.

Safe and efficient removal of all root filling materials from the root canal system without compromising radicular dentin structure is essential for optimal nonsurgical retreatment. The aim of this study was to evaluate and compare the incidence of dentinal defects caused during root canal filling removal using conventional, rotary, and reciprocating retreatment file systems. A detailed protocol explaining purpose and procedures of the study was submitted to the Institutional Ethics Committee and ethical clearance obtained. Sixty human maxillary permanent central incisors were collected and decoronated to 12-mm standardized length. The canals prepared up to a master apical file size F3 with Protaper hand files, obturated using AH plus sealer, examined under the stereomicroscope (×40 magnification): Group I: Control (n = 15), Group II: Conventional (n = 15), Group III: Protaper Universal Retreatment Files (n = 15), and Group IV: Reciproc Blue (n = 15). After instrumentation, teeth were sectioned at 3, 6, and 9 mm from the apex to evaluate the presence of dentinal defects under the stereomicroscope. Statistics were performed using the SPSS, version, 25 (SPSS Inc., Chicago, IL, USA). Initially, normality test was done using the Shapiro-Wilk test and data were not normally distributed followed by Kruskal-Wallis test. P < 0.05 is considered statistically significant. Maximum percentage increase in dentinal defects was observed in Protaper Universal Retreatment Files followed by Conventional method and Reciproc Blue. Significantly Reciproc Blue reduced the incidence of dentinal defects after root canal preparation.

Correlation between Cleft Width and Adjacent Teeth Inclination in Patients with Unilateral Cleft Lip and Palate Using CBCT: A Retrospective Study.

To evaluate buccolingual and mesiodistal inclination of cleft-adjacent maxillary permanent central incisors (U1) and canines (U3) in patients with unilateral cleft lip and palate (UCLP) in the late mixed dentition and to investigate the correlation between the alveolar cleft width and cleft-adjacent teeth inclination using cone beam computed tomography (CBCT). Observational cross-sectional retrospective study. Department of Orthodontics, Hama University Dental School. 32 patients with UCLP (22 boys, 10 girls; mean age = 10.53 ± 1.51 years). Buccolingual and mesiodistal inclination of maxillary central incisors and canines were measured on both sides. Then, the cleft and non-cleft sides were compared using the Wilcoxon signed-rank test. Pearson's correlation was used to explore the association between the alveolar cleft width and cleft-adjacent U1 and U3 buccolingual and mesiodistal inclination. The cleft-adjacent central incisors were significantly inclined lingually and distally compared with their non-cleft antimeres (P = .003, P < .001, respectively). The cleft-adjacent canines were significantly inclined buccally and mesially compared with their non-cleft antimeres (P < .001, for both). A positive correlation was found between the buccolingual inclination of cleft-adjacent U1 and the alveolar cleft width (r = 0.49, P = .004). Patients with UCLP demonstrated a significant lingual and distal inclination of cleft-adjacent U1 and a significant buccal and mesial inclination of cleft-adjacent U3. The buccolingual inclination of cleft-adjacent U1 tends to increase with increasing the alveolar cleft width; however, the correlation was weak.

Traumatic Dental Injuries Among 6 – 13 – Year - Old - School Children in Tikrit City

Background: An injury to both primary and permanent teeth and the supportingstructures is one of the most common dental problems seen in children. The extentof injury may vary from mild chipping of enamel to sever maxillofacial injury.The aim of this study was to find out the prevalence of traumatized anterior teethamong primary school children.Materials and methods: This study is a cross-sectional survey was carried outthrough clinical examination of anterior teeth among 720, 6-13 year old childrenenrolled in primary public schools of tikrit city. The prevalence of traumatizedanterior teeth was studied in relation to age, gender, type of injury in addition tothe cause of fracture. Recording and diagnosis of dental fracture were assessedaccording to Garcia-Godoy classification.Results: Males were more affected than females with odds ratio was 2.53 andstatistically was significant. The higher prevalence of dental trauma was recordedamong the age 10-11 year old. The permanent maxillary central incisors were themost frequent injured tooth and the most frequent type of fracture was the Class IIEnamel-Dentine, fracture without pulp exposure. Fall and playing were the mostfrequent causes of dental injury.Conclusion: data of the present study clearly show the need for dental healtheducation of children and their parents, so prevention programs are needed toimprove the dental health of Iraqi children.

Interceptive Management of Dilacerated Maxillary Incisor: A Case Report

Because of the intimate physical link between the primary tooth and the permanent tooth germ, traumatic dental injuries in the primary dentition might impair the development of permanent teeth, particularly in the maxillary front region. Dilaceration is a condition that is commonly encountered as a result of dental trauma and is defined as an abrupt deviation along the long axis of the tooth that can be visible between the crown, root, or both. Due to their location within the esthetic zone, such teeth present a special difficulty to the pediatric dentist. This article aims to describe a case of a 09-year-old female patient with an ectopic permanent maxillary central incisor, which was diagnosed as dilacerated after the radiographic examination. The case was managed with an interceptive approach using a Hawley palate.

Kvaal’s dental age estimation method applied to Brazilian adults—a cross-sectional test of validity

BackgroundDental age estimation of adults is a challenging procedure in forensic odontology. Radiographic techniques have enabled non-invasive age estimation of adults since the 90’s. Kvaal’s method based on the deposition of secondary dentin has demonstrated broad applicability in several populations. Population-specific studies with this method, however, are scarce in South America. This study tested the validity of Kvaal’s method in a sample of radiographs of Brazilian adults. The sample consisted of 192 periapical radiographs of 106 females (55.2%) and 86 males (44.8%) with ages between 20 and 80 years old. The permanent maxillary right (n = 84, 43.8%) and left (n = 108, 56.2%) central incisors were assessed based on Kvaal’s method.ResultsLin’s coefficient of concordance between estimated (EA) and chronological (CA) ages was 0.800 (IC95% = 0.753; 0.847) (p < 0.001). The mean difference between EA and CA was − 2.84 years (p < 0.001). For females and males, separately, the mean differences between EA and CA were − 3.82 years (CI95% =  − 5.38; − 2.25) (p < 0.001) and − 1.86 years (CI95% =  − 3.56; − 0.17) (p = 0.031), respectively. Underestimations were predominant and increased with time.ConclusionAfter 28 years of the original publication, Kvaal’s method remains reliable for non-invasive dental age estimation of adults, especially for the analysis of the permanent maxillary central incisors of Brazilian individuals.

Estimation of Crown Length, Root Length and Golden Divine Ratio in Extracted Permanent Maxillary and Mandibular Central Incisors: An In-vitro Study

Introduction: Human teeth serve many crucial functions, and aesthetics is one of these. Aesthetics and beauty are often correlated with the Golden Divine Ratio (GDR), a unique proportion hypothesised to exist in individuals or things with a harmonious appearance. Aim: To estimate the crown length, root length, and GDR in extracted human permanent maxillary and mandibular Central Incisors (CI) in the population of Tamil ethnicity from South India. Materials and Methods: This in-vitro cross-sectional study was conducted in the Department of Oral Pathology and Microbiology at Saveetha Dental College and Hospitals, Chennai, Tamil Nadu, India, from January 2023 to October 2023. The study included a total of 90 extracted human teeth from the Institutional tooth repository, out of which 27 were upper permanent CIs and the remaining 63 were mandibular CIs. Crown length and root length were measured using digital vernier calipers, and golden ratios were derived. Statistical Package for Social Sciences (SPSS) software version 26.0. was used for descriptive analysis and deriving means and standard deviation. Results: The average crown length, root length, and total length of upper CIs were 8.06±0.09 mm, 13.01±1.47 mm, and 21.06±2.36 mm, respectively. For the remaining 63 extracted permanent mandibular CIs, the mean crown length, root length, and total length were 7.85±0.66 mm, 12.6±0.97 mm, and 20.45±1.49 mm, respectively. The Root-to-Crown (R/C) ratio and Total Length to Root Length (T/R) ratio approximated 1.618 for both sets of teeth. Conclusion: The crown length of maxillary and mandibular CIs from the Tamil population followed the golden ratio in the present study.

Screening for the risk of canine impaction, what are the presumptive signs and how does it affect orthodontics? A cross-sectional study in France.

The treatment of impacted canines is a challenge for orthodontists. The availability of suggestive clinical signs has become crucial for treatment before the potential for evolution ceases. The main objective was to evaluate the prevalence of the suspected displaced canine (SDC) and to highlight factors easily identifiable from the oral examination. SDC was assessed on panoramic X-rays, on the basis of the angle with the median sagittal plane and the degree of overlap with the permanent lateral incisor. Its association with mesio-distal tooth and palate widths was assessed by univariate analysis. Next, the association of SDC with temporary tooth extraction, expansion and/or premolar extraction was considered using the same modality. In this retrospective study, the records of 292 patients aged 7 to 13 years were reviewed. SDC was detected in 39% of patients i.e., 28,8% of observed canines. Reduced coronal mesio-distal diameters of permanent maxillary central incisors, 8.7 ± 0.6 mm versus 8.8 ± 0.7 mm (p < .05), and first permanent molars, 10.0 ± 0.7 mm versus 10.2 ± 0.7 mm (p < .001), as well as reduced inter-molar width, 38.7 ± 2.7 mm versus 39.4 ± 2.9 mm (p < .01), were predictive factors. SDC led orthodontists to indicate extraction of maxillary primary canines, OR = 3,32 (p < .001) or even extraction of premolars, OR = 1,66 (p < .05). This study confirmed the interest of panoramic X-rays in detected canines at risk of SDC. Dental factors can be combined to make screening more reliable and predict impaction that makes orthodontics complex. Opinion n°21.131, dated 09.21.2021, retrospectively registered.

A computational model to assess the effectiveness of adhesive materials in restoration of crown-root fractures

Fragment reattachment for crown root fractures has become a routinely employed treatment modality with the advancements in adhesive dentistry. Among the majority of documented dental trauma cases, this specific fracture type is one of the most prevalent and difficult fracture kinds. Due to its complexity, these fracture kinds are often not computationally modelled. Moreover, there is a lack of literature to understand the effect of different adhesive materials, used to re-attach this particular fracture, on traumatic injuries. In our work, 3D models of the permanent maxillary central incisor tooth were developed using cone beam computed tomography image of a patient. This model was systematically modified to represent a prominent crown root fracture and subsequently re-attached computationally using three different adhesives. A biting force and a traumatic load were applied, and the induced stresses were studied across the healthy and treated tooth models and compared for three different adhesives used for re-attachment of fractured fragments. Tooth reattached with resin adhesive performed better in all the loading conditions that were considered in the study as compared to flowable composite and resin cement.

Root development differences between cleft-adjacent teeth on the cleft side in comparison to their analogs on the noncleft side in patients with nonsyndromic cleft lip and palate who received secondary alveolar bone grafting.

To assess differences in root development between the cleft side (CS) and noncleft side (NCS) for permanent maxillary central incisor and canine longitudinally in patients with nonsyndromic complete unilateral cleft lip and palate (cUCLP) who received secondary alveolar bone grafting (SABG) and to evaluate the effects of SABG on the acceleration of root development of these teeth. Permanent maxillary central incisors and canines of 44 subjects with nonsyndromic cUCLP who had all their cleft-related surgeries performed by the same surgeon were analyzed retrospectively from chart notes and radiographs. Panoramic and periapical radiographs at time point 1 (T1) (age, 7.55 years), at SABG (time point 2 [T2], 10.13 years), and a minimum of 2 years after SABG were studied. Root development rating scores on the NCS and CS were compared using paired t-tests and analyses of proportions. Mean root development score differences (NCS - CS) for canines and central incisors were greatest at T2 but diminished at time point 3 (T3). A larger proportion of teeth on the CS trailed the teeth on the NCS by at least 1 point at T2 than at T1 or T3, with the smallest proportion being observed at T3. The change in root development scores from T1 to T2 and from T2 to T3 showed relative CS acceleration from T2 to T3, indicating a catch-up of root development of cleft-adjacent teeth after SABG. Root development of cleft-adjacent central incisors and canines is slow in comparison with their noncleft analogs. Root development of these teeth accelerates following SABG.

Shear Bond Strength of Light Cured Resin Composite to Bleached Enamel Pre-Treated with Probiotic

Aim: To assess the Shear Bond Strength (ShBS) of light cured composite resin to bleached enamel pre-treated with probiotic. Materials and Method: 40 extracted permanent maxillary central incisors teeth were obtained, decoronated, mounted on resin blocks and grouped into four namely Group 1: Resin Composite restoration done without bleaching (positive control), Group 2: Vital bleaching is done with 40% hydrogen peroxide for 20 minutes followed by composite restoration (negative control), Group 3: Composite restoration delayed by 4 weeks after vital bleaching, Group 4: Composite restoration on bleached enamel pretreated with probiotic for 60 minutes. All the specimens were conserved for one day (24 hours) in deionized water and subjected to analysis for ShBS using cross head speed of 0.5 mm per minute. Statistical Analysis: Data was tabulated on excel sheet and analyzed statistically using SPSS software. Results: One way ANOVA statistical test showed a statistically significant difference in the mean ShBS values between the groups. Group 1 (positive control), Group 3 (delayed bonding), and Group 4 (probiotic) had considerably higher values of mean ShBS than Group 2 (immediate bonding). Conclusion: Probiotic pre-treatment on bleached enamel was able to effectively reverse the effect on ShBS of enamel treated with bleaching agent.

Aesthetic management of discoloured incisors in children.

Children can often present with discolouration of their teeth, commonly affecting their permanent maxillary central incisors. These opacities can often cause children to suffer with psychosocial and confidence related issues. With long waiting lists in secondary care, many children may face increased waiting times to receive aesthetic dental treatment. It is important that the dental professional is aware of the different causes of discolouration, the possible treatment options and when to consider a referral to secondary care.

Management of Bilateral Foreign Objects in Immature Permanent Maxillary Central Incisors: A Case Report

AbstractTraumatic dental injuries are more commonly seen in children and it often results in a complicated crown fracture. This case is unique as foreign objects in the form of hand-sewing needles were accidentally lodged bilaterally in both maxillary central incisors. A 13-year-old patient was referred to the Dental Clinics at the College of Dentistry, Jazan University with a complaint of broken needles in her front teeth and a history of trauma 3 years ago. Although the patient had the habit of using a hand-sewing needle to remove the lodged food particles from the open pulp chamber, she was not able to recollect the incidence of needle separations. Radiographic examination revealed radiopaque objects in the middle third of the right maxillary central incisor (#11) and in the apical third of the left maxillary central incisor (#21). Tooth #11 was clinically diagnosed as pulp necrosis with chronic apical abscess and tooth #21 as pulp necrosis with asymptomatic apical periodontitis. The micro tweezer from the broken instrument removal kit (Zumax Medical Co.,Ltd., Jiangsu, China) was used to retrieve the separated needle pieces under the operating microscope. The open apices were managed by the formation of mineral trioxide aggregate apical plug, and the procedure of root reinforcement was achieved by bonded resin cement and fiber resin posts. Finally, teeth were restored with dual cure core build-up composite material and direct composite veneers. Recommendations are made to treat complicated crown fractures, especially in children on priority to minimize the risk of foreign object impaction.

Purpose: The purpose of this study was to evaluate the prevalence of traumatic dental injuries (TDIs), as well as to evaluate different factors associated with TDIs among children and adolescents referred over three-year period to the Pamukkale University, Faculty of Dentistry, Department of Pediatric Dentistry, Denizli. &#x0D; Materials and methods: Dental trauma records of patients were reviewed regarding demographic data, affected teeth, type and etiology of TDIs, location and season of TDIs, time elapsed following TDIs and initial treatment, and treatment of TDIs.&#x0D; Results: Data associated with 258 patients, 471 traumatized teeth (primary teeth:152, permanent teeth:319) were analyzed. Both the primary and permanent maxillary central incisors were the most common affected teeth by TDIs. Primary teeth showed a higher frequency of periodontal tissue injuries (75.00%) and lower frequency of dental hard tissue injuries (21.05%) compared with permanent teeth (49.84% and 58.62%, respectively). Falls were the main etiological factor of TDIs both in the primary (73.68%) and permanent (58.31%) teeth. Only 27 patients (5.49%) were referred to the clinic within one hour following TDIs. &#x0D; Conclusion: Early management of TDIs is important to improve the prognosis of traumatized teeth and prevent post-traumatic complications. The low rate of early referral indicates the need for educational and preventive programs regarding TDIs among the patients, parents/legal guardians, and teachers.

Evaluation of mechanical properties of different polyetheretherketone endodontic post systems: an in vitro study

BackgroundSurvival of endodontically treated teeth depends on an efficient restoration of the missing tooth structure.ObjectivesThis study aimed to investigate the mechanical properties of different endodontic post systems.Materials and methodsHuman permanent maxillary central incisors (no.=58) were decoronated and root-filled. The specimens with prepared root canals were randomly divided into Group P: Polyether Ether Ketone (PEEK) post and composite core and Group C: custom-made post-core of PEEK. The cementation of the posts was carried out using self-adhesive resin cement. Nano-hybrid composite resin was used for core fabrication. After cementation, the specimens from each group were randomly divided into two subgroups according to the types of tests utilized: 14 from each group were for the fracture strength test, which was restored with IPS e-max CAD crown, and 15 from each group for the pullout resistance test. A universal testing machine was used for the test performance.ResultsThe fracture resistance test showed that the values significantly differed among tested posts (P = 0.013). Group P showed the highest fracture resistance. Group C exhibited higher mean pullout resistance values than the other group (P) (P = 0.059). In the two-piece PEEK post and composite core, the predominant type of failure was a core fracture, while in the one-piece PEEK post-core, most types of failures were either in the crown or in the post.ConclusionsThe prefabricated posts are more resistant to fracture than the custom-made posts, with fracture and displacement mainly of the core. In contrast, both showed similar pullout resistance.
